Details:
- Piapot First Nation will be distributing a $200 payment per band member (this includes your dependents).
- Payments for elders and seniors (over the age of 65) will be processed automatically. They do not need to apply.
Eligibility:
- Must be a Piapot First Nation band member.
- Must be 18 years of age to apply. Note – Anyone under the age of 17 must have a parent or guardian apply on their behalf.
- Must provide a photo of your health card as well as for your spouse and/or dependents.

- Save the completed form as a PDF file. Use a file name that includes your first initial and last name (e.g., JSmith.pdf).
- Attach the Following to an Email:

- A photo of your health card.

DEADLINE
To ensure you receive the bonus before December 25th, applications must be submitted by December 15th.
